Being at home and remaining independent is important for your health and happiness. Through ECHN’s home care services, we provide care for you or a loved one without having to leave your home. Care includes nursing medical services, therapy services and assistance with daily tasks.


For over 100 years, Visiting Nurse & Health Services of Connecticut (VNHSC) has been providing skilled home care services to the community. Dedicated to home health and community-based health care, VNHSC cares for families living in 25 communities throughout eastern and north central Connecticut.

Requests for care can be made by physicians and healthcare facilities, or even by a patient, family member or friend, a hospital or others.

Skilled Nursing Services

Home care services are community services for individuals 18 years of age and older that are temporarily bound to their home and are in need of skilled nursing services. Registered nurses specializing in home care facilitate necessary services and work closely with patients, family members and physicians to determine an individualized care plan aimed to keep patients safe, comfortable, and well cared for so they are able to achieve their optimal level of functioning.

Rehabilitation Therapy Services

Rehabilitation services are comprised of physical, occupational and speech therapies, all of which are provided in the patient’s home. Therapists work with patients after an injury or illness and develop a treatment plan designed to best achieve the goals of maximum functional independence. Depending on the nature of their diagnosis, patients may receive one therapy or a combination of services with other disciplines.

Medical Social Workers

Social workers provide counseling to the patient/family with problems related to illness or emotional stress. They provide information for a wide range of community services, help obtain financial assistance to help cover health care expenses, and can assist with alternative living arrangements, such as assisted living or nursing home options.

Home Care Aides

Home Health Aide services are delivered under the supervision of a registered nurse or licensed therapist and are provided when you need part-time assistance with personal care. Services are planned by you and your nurse or therapist and added to your plan of care. Typical duties include bathing, shampooing hair, changing bed linens and assisting with other activities of daily living.

Hospice Home Care 

Visiting Nurse & Health Services of Connecticut’s Hospice Program offers terminally ill patients an alternative to hospital or nursing care and seeks to enhance the life of the terminally ill patient and his/her family, enabling them to live each day to its fullest. Working as a team, physicians, nurses, social workers, home care aides, clergy and volunteers give care in the home, allowing patients to be with their family and friends in familiar surroundings with control over schedules and environment. Bereavement support is provided to family and friends for at least one year after a loved one passes away.

Palliative Care 

Palliative care provides symptom management and pain control for those patients experiencing both physical and emotional suffering related to a disease process which may or may not be life threatening.
