The Diabetes Self-Management Program has been recognized for excellence by the American Diabetes Association, and ECHN has been honored as one of its “most influential partners” in the fight against Diabetes in Connecticut. This program supports and inspires individuals to live full happy lives and includes educational materials, instructions and group education classes.
Led by the Centers for Disease Control and Prevention, PreventT2 is a proven system to help identify the signs of diabetes early and even prevent or delay the onset of type 2 diabetes.
ECHN’s gestational diabetes program has been recognized for excellence by the American Diabetes Association repeatedly since 1996 and has been honored by the American Diabetes Association as one of its “most influential partners” in the fight against diabetes throughout Connecticut. Certified diabetic educators work with pregnant women to create a personalized self-management plan that meets individualized needs and lifestyles. The focus is on creating a personalized care plan for you that includes ample support and education.
