In-Home Primary Care
ECHN Medical Group is pleased to offer In-Home Primary Care Services for qualifying patients who have difficulty traveling to their primary care provider's office. This program is designed to bring high-quality, compassionate medical care directly to your home, ensuring you receive the personalized attention you need in a comfortable and familiar environment.
Who is Eligible?
In-Home Primary Care Services are available to patients who meet the following criteria:
- Have an established primary care provider with ECHN Medical Group
- Experience mobility challenges, chronic illness, or other medical conditions that make it difficult to attend in-office visits
- Require ongoing primary care management at home
Please note: Patients must be referred by their ECHN Medical Group primary care provider to be considered for in-home services.
