Palliative care can be provided at the same time as treatment, it aims to prolong life for as long as possible. Often referred to as a bridge program, our compassionate caregivers provide emotional support to help prepare individuals who might not yet need – or are prepared for – hospice care.

Patients with a life limiting or chronic debilitating illnesses can benefit from palliative care services, and will receive comfort measures, family support and continued treatment options for as long as needed. We provide services and treatment options for a wide variety of illnesses to enhance the quality of life and positively influence the course of any illness, including but not limited to:

  • Cancer patients who may or may not be receiving chemotherapy or radiation treatments
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure (CHF)
  • Dementia
  • Dialysis patients
  • Endstage neurological disorders

Palliative Care Team

An interdisciplinary team provides palliative care so patients and their families have access to numerous medical and emotional support services. The team includes:

  • Bereavement coordinator
  • Home health aides to assist with personal care
  • Medical social workers for counseling
  • Nutritional support
  • Palliative medical director for consultation
  • Palliative certified and trained nurses to evaluate and manage pain and symptoms
  • Patient’s physician to oversee medical care
  • Physical, speech and occupational therapists
  • Spiritual care coordinator
  • Transitional coaches
  • Volunteers trained in pain and symptom management and supportive care to provide companionship and respite care

Palliative Care is covered under the home care benefit of most insurance plans when the patient is eligible for homecare.
