Diabetes PreventT2 Program

Did you know that having prediabetes means your blood sugar level is higher than normal, increasing your risk of getting Type 2 diabetes? Without weight loss or moderate physical activity, many people with prediabetes can develop Type 2 diabetes within 5 years.
Type 2 diabetes is a serious condition that can lead to health issues such as heart attack, stroke, blindness, kidney failure or loss of toes, feet or legs. People with prediabetes who take part in a structured lifestyle change program can reduce their risk of developing type 2 diabetes by 58%. For people over 60 years old, it is reduced by 71%.
About Our Program
PreventT2 is a 12 month, small-group coaching program that uses a research-based CDC approved curriculum that has been proven to be an effective lifestyle change program to help delay or prevent type 2 diabetes.
A trained lifestyle coach leads the program. You will work closely with this coach to learn skills needed to make lasting changes. You will learn how to healthily lose weight, increase physical activity and manage stress.
You will also be joining a group of people who are facing similar challenges and together celebrate successes and overcome obstacles.
A year might sound like a long time to commit however, investing 24 hours of your life can bring positive changes to your health by creating new habits, gain new skills and build confidence.

What is the Benefit of Being Part of the PreventT2 Program?
As part of the PreventT2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes with no cost to you. These changes include losing a modest amount of weight, being more physically active and managing stress by being part of a group that provides support. The group will be other people who are facing similar challenges and trying to make the same changes as you are. Together you can celebrate successes and find ways to overcome obstacles.
What Is the Format of the PreventT2 Program?
Groups meet for a year — 3 times a month on the same day and time in month 1-6, then once a month for the second 6 months to maintain healthy lifestyle changes. During each session, your lifestyle coach will teach a lesson and lead a group discussion. For example, you will learn how to:
- Eat healthier
- Add physical activity to your life
- Manage stress
- Stay on track when eating out
- Understand your responses to food
- Stay motivated
- Solve problems that can get in the way of healthy changes
Program Eligibility
The following criteria must be met for participation:
- 18 years of age or older
- BMI 25 kg/m2 or over (23 kg/m2 or over if Asian American)
- No previous diagnosis of Type 1 or Type 2 diabetes
- Previous gestational diabetes (not currently pregnant)
In addition, you must also have one or more of the following:
- HbA1C: 5.7%–6.4%
- Fasting plasma glucose: 100-125
- 2-hour plasma glucose (after a 75gm glucose load): 140–199mg/dL
- Previous diagnosis of gestational diabetes (may be self-reported)
If you do not meet these criteria, take the diabetes risk test to see if you are pre-diabetic.
