Having prediabetes means your blood sugar level is higher than normal, increasing your risk of getting Type 2 diabetes. Without weight loss or moderate physical activity, many people with prediabetes can develop Type 2 diabetes within 5 years. Type 2 diabetes is a serious condition that can lead to health issues such as heart attack, stroke, blindness, kidney failure or loss of toes, feet or legs.
ECHN is proud to offer the Prevent T2 Program to community members for FREE that are 18 years or older, have a BMI that is 25 or more (if you are Asian then 23 and more) and a glucose test or A1C that is categorized as prediabetes.

What is the Prevent T2 Lifestyle Change Program?
Prevent T2 is a 12 month, small-group coaching program that uses a research-based CDC approved curriculum that has been proven to be an effective lifestyle change program to help delay or prevent type 2 diabetes.
A trained lifestyle coach leads the program to help you change certain aspects of your lifestyle, like eating healthier, reducing stress and getting more physical activity. The program also includes group support from others who share your goals and struggles.

• It’s not a quick fix
• It’s a year-long program focused on long-term changes and lasting results
A year might sound like a long time to commit however, investing 24 hours of your life can bring positive changes to your health by creating new habits, gaining new skills and building confidence.
What is the Benefit of Being Part of the Prevent T2 Program?
As part of The Prevent T2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes with no cost to you. These changes include losing a modest amount of weight, being more physically active and managing stress by being part of a group that provides support. The group will be other people who are facing similar challenges and trying to make the same changes as you are. Together you can celebrate successes and find ways to overcome obstacles.
What Is the Format of the Prevent T2 Program?
Groups meet for a year — 3 times a month on the same day and time in month 1-6, then once a month for the second 6 months to maintain healthy lifestyle changes. During each session, your lifestyle coach will teach a lesson and lead a group discussion.
For example, you will learn how to:
• Eat healthier
• Add physical activity to your life
• Manage stress
• Stay on track when eating out
• Understand your responses to food
• Stay motivated
• Solve problems that can get in the way of healthy changes
