Your privacy is important to us. This notice explains our online information practices and how information you may provide is collected and used.
 
On some pages, you can send email messages or request publications via email. The types of personal information collected on these pages could include your email address, name, mailing address or phone number. We use the information you provide about yourself when sending an email message only to respond to the request. We do not share this information with outside parties except to the extent necessary to respond to the request. We use return email addresses to answer the email we receive. Such addresses are not used for any other purpose and are not shared with outside parties. We do not use or share the personally identifiable information provided to us online in ways unrelated to the ones described above.
 
The information provided on this website is intended for your general knowledge and is not a substitute for consulting your personal physician. ECHN has no control over such sites and does not endorse them and is not responsible for any content, information or materials related to any such site. Privacy Practices

Effective Date: March 29, 2014

This Notice of Privacy Practices (the “Notice”) is meant to inform you of the uses and disclosures of protected health information that we may make. It also describes your rights to access and control your protected health information and certain obligations we have regarding the use and disclosure of your protected health information.

Your “protected health information” is information about you created and received by us, including demographic information, that may reasonably identify you and that relates to your past, present or future physical or mental health or condition, or payment for the provision of your health care.

We are required by law to maintain the privacy of your protected health information. We are also required by law to provide you with this Notice of our legal duties and privacy practices with respect to your protected health information and to abide by the terms of the Notice that is currently in effect. However, we may change our notice at any time. The new revised Notice will apply to all of your protected health information maintained by us. You will not automatically receive a revised Notice. If you would like to receive a copy of any revised notice you should access our web site at www.echn.org, contact our hospitals, or ask at your next appointment.

We participate in Connie, the statewide health information exchange. 

We encourage you to learn more about Connie at www.conniect.org. 

As permitted by the Health Insurance Portability and Accountability Act (HIPAA) for treatment, payment, and operations, your health information will be securely shared with the statewide health information exchange. Our participation is mandated by the State of Connecticut. Participation helps your providers have faster access to your health information to make more informed decisions and enable them to best coordinate your care.  

You may opt-out and disable access to your health information available through Connie by calling or completing and submitting an Opt-Out form to Connie by mail, fax or through their website at www.conniect.org. Public health reporting and controlled dangerous substances information, as part of the Connecticut Prescription Monitoring and Reporting System (PMP), will still be available to providers.
