Obtaining Your Medical Records
If you need records from a physician, you will need to contact that physician's office to make arrangements. If you'd like records from services delivered at one of our facilities, please call our Health Information Management Department at (860) 533-2930, ext. 6433. Due to the number of calls received, you may need to leave your name and phone number so someone can return your call. You also may download a copy of our Release of Information (ROI) form by clicking here. Just complete the form and mail it to the facility where you were treated, and you will be contacted when your records are available.
About Our Health Coordinator
Kathy Porter, RN, BS, ECHN's Women's Health Coordinator, serves as a resource nurse and health system navigator on behalf of our female clients, physicians, and physician office staffs.
Kathy assists women in accessing the appropriate services within ECHN, with special emphasis on quick and timely access to breast health services. She also works to increase public awareness about the full range of women's health services that ECHN has to offer, through health education efforts.
The Women's Health Coordinator position is made possible through the generous support of the NewAlliance Bank Foundation, which awarded a grant to ECHN for this purpose in April 2004.
Kathy Porter received her Associate's Degree in Nursing from Northern Essex Community College, Haverhill, Mass., and received a B.S. in professional arts, with a concentration in health care administration from St. Joseph's College of Maine. She recently received her certification as a breast care specialist.
Kathy first joined ECHN in 1993 as an Intensive Care Unit nurse at Manchester Memorial Hospital. She subsequently served as a Cardiac Rehab nurse at MMH from 1995 to 1998, and from 1998 to 2003 she was the assistant manager for the MMH Cardiac Rehab Program/Stress Lab. From November 2003 to her present position, Kathy was the manager of ECHN's Cardiac Rehab/Stress Lab.
Kathy can be reached by telephone at the Women's Center for Wellness, (860) 533-4646, or via e-mail, kporter@echn.org.
